In meshing process with hexa elements fairly often leads to formation of deformed elements, which also have interpenetration. But no mesages about bad shape of elements. Usually it happens around the holes. Problem is in version 11.2.
Does anyone know how to solve this problem? How to prevent it?
If you open attached MODFEM, delete mesh, and mesh model again, result will be bad.

Well your geometry is very basic.
Follow these steps:
1/ Create a Washer + Pad around the hole
2/ create a 2D mesh on one surface.
3/ Mesh / Extrude / Elements , select 2D elements to create HEXA elements.
4/If you want to attach your mesh to the geometry then use Modify / Associativity command.
You can also surface mesh first, and tell the quad mesher to lay down some nice edge quads -
Mesh - Geometry - Surfaces
Hit the "More Options" and set "Quad Edge Layers" to 2
Yes to "Ok to mesh with Plot-Only Elements"
You will now have a surface mesh with nice quads around the holes.
Now Mesh - Geomtry - Hex Mesh Solids
Since the surface are already quad meshed, they don't get meshed again, the hex mesher use the existing ones and creates a nice hex mesh -
