you need to delete your SEECTransLog.xml
found in your SEEC Cache location
I don't know the actual root cause. seems like at times transactions fail or hang in some way. nothing you did didn't do that caused it.
As users of Insight we also get hung up, lost transactions, cache mismatches on a random basis. I suspect network hiccups as a cause but that's just a guess.
